What you should know...
1. The Department of Health is in charge of complaints about Audiologists and Hearing Instrument Dispensers. They have to follow many rules and steps before they can answer a complaint and maybe fix it.
2. It can take the Department of Health up to 3 years to solve your complaint and you may not hear from them at all. If you want refunds, then the Department of Health says that you should think of private ways to get the refund. Private ways might mean conciliation court.
3. Do you still want to file (send) a complaint?
- Get a copy of the complaint form for audiologist/hearing isntrument dispensers.
- Call the Department of Health at 651-201-3729 or 1-800-657-3837 (v). AND/OR
- go to the Deparment of Health’s website: print out a copy
- Fill it out.
Don’t worry if you printed your own copy. The Deparment of Health says that you can use the audiologist complaint form for complaints about hearing instrument dispensers too. In the future the Department of Health will change the complaint form so you will see only ONE complaint form for audiologists and hearing isntrument dispensers.
